In partnership with Look Ahead Hammersmith & Fulham, we deliver tailored homecare and support services each week across sites including Irving Road, Lena Gardens, and Kwanzaa House. These locations cater to individuals facing complex challenges such as mental health conditions, substance misuse, and homelessness. At VCP, our role extends far beyond traditional care delivery. We adopt a holistic, multi-disciplinary approach that addresses not only physical care needs but also emotional and social wellbeing.
By working closely with Look Ahead, we help to create and implement person-centred support plans designed to stabilise, rehabilitate, and support reintegration into the community. Our dedicated care team is trained in trauma-informed care and behavioural interventions, ensuring our services are compassionate, adaptable, and responsive to each individual’s unique circumstances. Through daily visits, therapeutic activities, and regular engagement, we empower service users to develop life skills, improve health outcomes, and achieve greater independence.
Our work has made a real difference for residents of Irving Road, Lena Gardens and Kwanzaa House, many of whom have struggled with a history of non-engagement with support services. VCP’s therapeutic and person-centred approach, built on patience and understanding, has helped to build trust and rapport with these individuals, supporting them to engage in the local community. As a result, service users who may have previously resisted support are now engaging with services that address both their immediate needs and long-term goals.
The impact of our work is transformative. By taking the time to understand each individual’s experiences, triggers, and needs, we deliver tailored, non-judgmental care that facilitates gradual and sustainable change. Our service users have shown notable improvement in their wellbeing and living conditions. Many have developed healthier routines, better managed their conditions, and engaged more with community resources.
